The first wide spread video streaming method was called CuSeeMe. This came out in the mid 90's when 56 Kbs was considered blazing fast. Nasa broadcast video from Shuttle missions and educational presentations using CuSeeMe. Several colleges also used CuSeeMe to broadcast their student run terrestrial radio and television stations.
You can read a online version of a book written about CuSeeMe here : Internet TV with CUSeeMe.
P2P ( or Peer to Peer ) was not always considered the Holy Grail for online multimedia broadcasting. IP Multicast ( or MBone ) was once considered the Gold standard for universal multimedia data delivery. Unfortunately, this technology was never fully implemented on the open Internet.
